TO: ALL PERSONS HAVING CLAIMS OR DEMANDS AGAINST THE ABOVE ESTATE
Within three months from the time of the first publication of this notice, you are required to file with the Clerk of the Circuit Court of Clay County, Florida, Probate Division, the address of which is 825 N. Orange Avenue, P. 0. Box 698, Green Cove Springs, Florida, 32043-0698, a written and verified statement of any Claim or demand you may have against the Estate of KATHIE ROSE GILLMAN, Deceased.
Each claim must be in writing and must indicate the basis for the claim, the name and address of the creditor or his agent or attorney, and the amount claimed. If the claim is not yet due, the date when it will become due shall be stated. If the claim is contingent or unliquidated, the nature of the uncertainty shall be stated. If the claim is secured, the security shall be described. The claimant shall deliver a copy of the claim to the clerk who shall furnish the copy to the Personal Representative.
ALL CLAIMS AND DEMANDS NOT SO FILED WILL BE FOREVER BARRED.
