GREEN COVE SPRINGS – The Florida Department of Health in Clay County urges parents and guardians to beat the rush and schedule Back to School Immunizations early.
The Bear Run Clinic will offer all required vaccinations and documentation for students within the Clay County School District, including COVID-19 vaccines, by appointment only. Be advised that both the Bear Run Clinic and private offices have limited appointment slots for Back to School vaccines. Please do not wait until the last minute to get your child’s required vaccines before school starts. Avoid the Back to School rush and make an appointment as soon as possible.
Students entering seventh grade require a T-DAP, and all students require an updated Florida Certification of Immunization form documenting receipt of vaccinations.
This form is required whether students return to a brick and mortar school, participate in virtual learning or a hybrid of both. Please note that limited appointment slots may delay the entry of your child into school for the 2021-2022 school year.
Bring all current immunization records and photo identification. Children younger than 18 years need to be accompanied by a natural or adoptive parent, legal custodian, legal guardian or person who possesses a power of attorney to provide medical consent for the minor.
A step-parent, grandparent, an adult sibling of the minor, an aunt, or an uncle can bring the minor child in for immunizations if they have written consent from a natural or adoptive parent, legal custodian, legal guardian or a person who possesses a power of attorney to provide medical consent for the minor and we can reach that person by phone.
