Middleburg man dies in Putnam crash
GRANDIN – A Middleburg man died in a traffic crash between two dump trucks at the Vulcan Grandin Sand Plant on State Road 100, according to The Florida Highway Patrol.

FHP said William Brathwaite, 59, of Middleburg, was driving a dump truck Jan. 28 at 5:54 p.m. heading eastbound on SR 100 when the front of his dump truck collided with the left side of a trailer being pulled by a dump truck driven by Willie Williams, 58, of Live Oak.

According to the FHP investigation, Brathwaite’s dump truck rotated before coming to a final rest facing north with its left wheels off the ground. Williams’ vehicle came to final rest on SR 100 facing northwest.

Williams was transported to UF Shands hospital in Gainesville for treatment of minor injuries, while Brathwaite was dead at the scene.

DUI Manslaughter charge in 2017 fatal crash
GREEN COVE SPRINGS – A Middleburg man remains in the Clay County Jail following his Jan. 23 arrest for DUI Manslaughter.

According to the arrest warrant from the Florida Highway Patrol, Jason Craig Osborne, 42, ran his 2009 Pontiac G6 sedan head-on into a 1996 Ford Ranger truck on Nov. 9, 2017 at the intersection of State Road 21 and County Road 352, about three miles north of Keystone Heights.

FHP pronounced Timothy Curtis Meek, 58, of Keystone Heights, dead at the scene. Meek was driving northbound on SR 21 when Osborne’s vehicle veered into the northbound lane and struck Meek’s truck head-on.

Prior to the crash, witnesses told FHP they saw Osborne driving erratically, traveling off the road and passing into oncoming lanes.

The night of the crash, which occurred at 10:20 p.m., Osborne was taken to Orange Park Medical Center where officials took a voluntary blood sample that would be sent off to the Florida Department of Law Enforcement Crime Lab for testing.

The lab tests, which FHP received on Jan. 1, 2018, showed a toxic level of the Schedule IV Narcotic Alprazolam in Osborne’s bloodstream, which caused him to be impaired.

Osborne remains held with his bond at $75,003.

Battery charge for striking senior
GREEN COVE SPRINGS – A Jacksonville woman faces charges of battery on a person 65 years or older.

According to police, Helen Elaney Seroki, 26, hit a man in the face at the Clay County Courthouse on Jan. 29 at approximately 4 p.m.

The Clay County Sheriff’s Office reported that Seroki and the man are “in a custody dispute over her child.”

According to the police report, witnesses told police the man’s right ear was swollen after the alleged attack.

At press time, Seroki remains at the Clay County Jail with a bond set at $7,503.


Man caught with burglary tools
ORANGE PARK – A Jacksonville man faces charges of grand theft auto and possession of burglary tools.

According to the police report, Clay County Sheriff’s officers interviewed Jonathan Edwin Simms, 31, on Jan. 29 in relation to a separate burglary.

CCSO reported that an investigation revealed Simms was driving a stolen vehicle.

At press time, Simms remained in the Clay County Jail where his bond is set at $40,009.
